If you want a custom thumbnail on youtube, I believe that you have to assign the thumbnail image by following youtube's instructions and using youtubes account controls. If you want to use one on facebook for an ad video, you have to set it up using facebooks special technique. I believe that the same goes for twitter, powerpoint, quicktime, and most other programs, browsers and sites that allow custom thumbnails.
